Case 1

Male patient in his 30’s presents with acute breathlessness for 10 days suggestive of an acute PE. He remains tachycardic but other observations are normal. 

  1. Why is he still tachycardic 10 days on?
  2. Is something else modifying his risk?

Case 2

Male patient in his 20’s presents with acute chest pain. Initially anticoagulated for ?PE elsewhere and returned for SDEC review after having a CTPA 3 days later. CTPA was negative for PE and reported to be unremarkable. Review of history indicated pain was sharp with an early-diastolic murmur present. 

  1. Key history and assessment raises the suspicion and PoCUS question!

Case 1

Middle aged man brought into Resus with sever breathlessness. ITU in attendance due to hypoxia and hypotension. Was difficult to ventilate and too unstable to move for imaging. ITU raised the below questions for PoCUS help.

  1. Does he have a tension pneumothorax, before we intubate?
  2. Is there a cardiogenic cause for this?

Case 2

Middle aged man presents with over a week of breathlessness, fever and right chest sided pain. Of note, he reported swinging temperatures, anorexia and significant lethergy also.

  1. History suggests more complex presentation than a simple pneumonia.
  2. …always scan through a 3D structure.

Abdominal-Renal Cases

Case 1

Middle aged man referred to SDEC following weeks of lethergy, anorexia and weight loss. Recent bloods in the community showed a new acute kidney injury and a few days prior to SDEC attendance, patient reported haematuria. 

  1. PoCUS supports AKI assessment and can significantly impact the course of things in only a few minutes…!

Gentleman referred to SDEC with non-specific symptoms for a few weeks and feeling generally unwell. Significanlty, rence blood tests showed a newly derranged liver function test. His right upper quadrant was diffusly uncomfortable.

Elderly gentleman who is an arteriopath presented to SDEC with unilateral leg swelling. There was no complaint of leg pain and the period of leg swelling was unclear.
